肝脏损伤

腹部外科中需要紧急处理的一种严重损伤。国内资料显示肝脏损伤在腹部损伤中居第4位,仅次于脾、肾、小肠的损伤。肝脏是腹腔内最大的实质性脏器,位于右侧膈下和季肋深面,受到胸廓和膈肌保护,一般不易受伤;但其质脆、血运丰富及较固定,易受外来暴力或锐器刺伤而引起破裂出血。肝脏损伤常见原因:战时绝大多数为火器伤;平时除火器、刃器伤外,多为交通、工业事故所致的挤压伤或钝性伤。根据致伤原因可将肝脏损伤分为开放性损伤与闭合性损伤两大类。按其损伤的病理性质可将闭合性损伤分为肝包膜下血肿、肝的真性破裂与肝中央破裂。主要临床表现有腹腔内出血、休克和腹膜刺激征,开放性损伤还可见伤口。治疗原则应及早诊断,早期手术。少数症状不明显的小的刺伤或小的包膜下血肿或无大量出血表现者,可在密切观察下,谨慎地采用非手术治疗;如病情出现恶化则应立即手术探查。影响其死亡率的因素有受伤后是否得到及时的抢救和治疗、肝脏损伤的类型及严重程度、是否有并发损伤等。
