单纯股骨滑车软骨损伤

当屈膝跪倒或者膝外翻位屈伸突然剧烈研磨时,可使股骨滑车被顶撞或捻转损伤,引起软骨龟裂、开窗样骨折或缺损,晚期继发软骨软化、溃疡缺损或软骨下囊泡形成。受伤时出现患膝酸痛,稍后痛减轻或消失,数周或数月后逐渐出现半蹲痛,可伴有关节肿胀。推髌伸膝抗阻试验阳性。膝于全屈位时可在滑车上触到压痛点。关节镜检查有助于明确诊断。早期可使用护具、股四头肌等张等长肌力锻炼和理疗。严重影响跳跃或下蹲者,可手术切除软骨病灶、软骨下骨质钻孔,术后不负重屈伸练习膝关节。
