多发生于腰骶部，也可见于背部。肿物为圆形，可能大如橘子，里面除脑脊膜和脑脊液外，还有神经组织。外面盖有很薄的皮肤，在中心区可能只盖有半透明的脑脊膜。在新生儿，有时只有肉芽组织，很容易有溃疡形成。脊髓组织进入肿物的上部，神经纤维广泛分布于肿物，然后由其下部又回到椎管内的脊髓部分。发育不良的脊髓、神经、脊髓膜、椎骨弓、肌肉常和皮肤连在一起。有些患儿呈脊髓外翻畸形。本病病人几乎都有下肢瘫痪和大小便失禁，有些并发脑积水。骶部脊髓脑脊膜膨出无下肢瘫痪，有大小便失禁。
