《精神疾病诊断和统计手册》仍iag-nostic and Statisrieal Manual ofMental刀15口rder:，DSM)美国精神病学会1 952年公布了《精神疾病诊断和统计手册》第一版(D SM一I)。当时受阿道夫·麦尔(Ad。ir Moyer)的精神生物学学说的影响，认为精神疾病是个体对环境的反应，对各种精神疾病均称为“反应”和“精神分裂症反应”、“神经官能症反应”等;并应用弗洛伊德的精神分析学说来解释神经官能症和人格障碍。尽管DSM一I对美国精神病学界产生了较大影响，但未被广泛接受。1968年公布了DSM一n，将精神疾病分为十类，取消了病名后的“反应”字样，列入了儿童、青少年类别，鼓励对疾病做多种诊断，即使两者有因果关系，如酒癖和继发性抑郁。DSM-n最主要的价值在于促进了国际间的研究和交流。1980年公布了DSM一111，对每种疾病提出了操作性诊断标准，提倡多轴诊断;对病因不明的疾病采用了描述性分类原则。DSM一m以全新的面貌出现，被认为是美国精神病学界的一个重要转折点，引起了世界范围的高度重视，被翻译成许多文字，为多种教科书引用。但在应用过程中，存在着许多诊断系统上的不稳定性，且在一些实例中存在诊断标准的不完全清晰性。1987年公布了DSM一111的修改版本(D SM一111一R)，在分类学原则、疾病概念、诊断方式上均与D SM-m相同，只是对某些细节进行了增补、删减、合并、分离和转换。在此基础上，美国精神病学会于1994年公布了DSM一W。在研究方法上，依据对大量文献和数据进行综合性和系统性审查、分析和实验，注重与IcD一10(国际疾病分类)的相容性，接受许多国际专家和相关组织的评审，增强了其科学性和跨文化的实用性:在障碍概念上，增加了症状临床意义的内容，提高了精神障碍诊断标准的阂限，使281种Axi:I和Axis 11的障碍类别中有 130种障碍包括了关注临床意义的症状标准，突出了生物一心理一社会的诊断模式;在分类学原则上，进一步突出了现象学的描述性原则，例如不再使用器质性精神障碍病原学诊断这一术语，也使DSM一W的诊断方式注重症状描述性特征，这些调整提高了DSM一W的障碍诊断系统的覆盖，使原在DSM一111一R中不能诊断或只能在“其他障碍”类别中诊断的精神障碍有了明确的诊断;补充了17种“新”的精神障碍，消减或合并了8种精神障碍。另外，DSM一W在文化的适应性上作了特别的考虑，提供了系统地评估和判断不同文化背景中的来访者的诊断模式。 (赵亚志郭金山撰刘协和市)
