球旁器(juxtaglomerular apparatus)又称球旁复合体，主要由球旁细胞、致密斑和外系膜细胞组成。
1、球旁细胞(juxtaglomerular cell) 是入球小动脉靠近血管极处，由中膜平滑肌细胞特化而成的上皮样细胞。细胞呈立方形，核圆居中，胞质弱嗜碱性，球旁细胞能分泌肾素，肾素在血液内经过一系列生化反应，可使血压增高。
2、致密斑(macular densa) 是远曲小管近血管极一侧的管壁上皮细胞特化而成的椭圆形结构。致密斑细胞呈高柱状，排列紧密，核呈椭圆形，位于细胞的顶部。一般认为致密斑是一种离子感受器，能感受远曲小管内钠离子的浓度，当钠离子的浓度降低时，将信息传给球旁细胞，促进球旁细胞分泌肾素。
3、球外系膜细胞(extraglomerular mesangial cell) 又称极垫细胞，位于入球小动脉、出球小动脉和致密斑之间的三角形区域内。细胞体积较大，有小突起，染色较淡。功能尚不清楚。
