纵隔肿物
mediastinal mass
位于纵隔内占位性病变的统称。X线片上可见纵隔内肿物。通过观察肿物的位置、形状、内部结构和密度的改变、边缘状况和有无扩张性搏动等功能对定性诊断有某些作用。畸胎类肿瘤好发于前下纵隔，内部可见钙化及骨骼、牙齿等致密影，CT还可辨别瘤体内有无脂肪成分。神经源性肿瘤好发于下后纵隔及上纵隔后部，常和脊柱影像重叠，正位若和心影相重叠容易漏诊，有时可发现局部椎体、肋骨和神经孔被压迫破坏等征象。淋巴瘤多分布在纵隔内气管和支气管周围的淋巴组织，形成多个淋巴结肿大的波浪状轮廓。气管支气管囊肿多位于气管支气管分叉水平，形如水滴状，CT示液体密度(10HU±），且随访多年变化不著。食管裂孔疝则位于下中纵隔，食管钡餐透视有利于诊断。CT和MRI可更好地显示纵隔肿物的形状、内部结构及与毗邻脏器的关系，对定性诊断可提供更多信息。
