胃十二指肠溃疡大出血

系指溃疡病引起的明显出血症状。5%～10%需外科手术治疗。大出血多为溃疡侵蚀基底中等动脉破裂的结果。多位于胃小弯或十二指肠后壁。主要症状为大量鲜血便、黑便，或伴急性大呕血。失血量超过800ml时，可出现休克现象。腹部无明显体征。血红蛋白下降。有典型溃疡病史者，伴上述表现可诊断。手术指征：①出血短期内休克；②短期输血600～900ml后，情况无好转；③正在内科溃疡病治疗期间大出血；④60岁以上大出血；⑤并发幽门梗阻或穿孔。手术包括溃疡在内的胃大部分切除术。
