因呼吸功能障碍所致肺泡换气减少，机体内二氧化碳潴留，二氧化碳分压增高，血碳酸浓度上升，pH值下降而引起一系列病理生理改变。见于以呼吸道阻塞为主的喉炎、气管异物、重症肺炎、肺水肿、支气管哮喘、呼吸肌麻痹等。此时CO2堆积，PaCO2上升，pH值降低，由于代偿作用，HCO3-增加，故CO2结合力增高。临床上，轻者可感倦怠、头痛、兴奋、当PaCO2＞10kPa(75mmHg）时，可出现CO2麻痹，表现为嗜睡、半昏迷以至昏迷、抽搐、肌肉瘫痪。积极治疗原发病，改善通气和换气功能，排出多余的二氧化碳是防治本病的根本。
