起病缓慢，病程较长，病初常无发热史，有长期使用抗肿瘤或免疫抑制药物史。颅内压常呈显著增高，头痛剧烈，与脑膜炎其他表现不相平行，面神经多不侵犯，但常有视力障碍及视乳头水肿，症状有时自行缓解。脑脊液常呈蛋白细胞分离，糖量显著降低，墨汁染色可找到厚荚膜圆形发亮的菌体，在沙氏培养基上有新型隐球菌生长。
