| 进行性肌营养不良 progressive myodystrophy 一组与遗传有关的原发性肌肉变性病。大致可分为以下6种类型。①假性肥大型肌营养不良。为性联隐性遗传病，绝大多数见于男孩。其基因位点位于Xp21上，因缺乏此基因产物——肌营养不良素而发病。于学龄前发病，走路呈鸭行步态，腰椎前凸，从卧位站立过程中出现高尔斯氏征。多数伴肌肉假性肥大，尤以腓肠肌突出，亦可见翼状肩胛。血清CpK极度增高，肌电图呈肌原性损害。如出现心肌损害，25岁前死亡，称为迪谢纳氏型；如进展缓慢，可成活25年以上，称为贝克尔型。②面、肩、肱型肌营养不良。为常染色体显性遗传病。萎缩肌肉以面肌、肩胛带和上臂肌肉为主。③肢带型肌营养不良。属常染色体隐性遗传。萎缩肌肉主要分布在肩胛带和骨盆带肌肉。④远端型肌营养不良。以四肢远端肌萎缩为主。⑤眼肌型肌营养不良。以眼外肌瘫痪为主。⑥眼咽型肌营养不良。以眼外肌和咽喉肌瘫痪为主。 对该病尚无特效疗法，只能采用支持和对症治疗，晚期应治疗和预防继发感染和心力衰竭。 |
