卵巢恶性肿瘤转移途径
卵巢恶性肿瘤的外观常常呈局限状,但在腹膜、大网膜、腹膜后淋巴结、横膈等部位已有亚临床转移。其转移途径主要通过直接蔓延及腹腔种植、瘤细胞可直接侵犯包膜、累及邻近器官,并广泛种植于腹膜及大网膜表面。淋巴道也是重要的转移途径,有3种方式:①沿着卵巢血管的走行、从卵巢淋巴管向上达腹主动脉旁淋巴结;②从卵巢门淋巴管达髂内、外淋巴结,经髂总至腹主动脉旁淋巴结;③沿圆韧带入髂外及腹股沟淋巴结。横膈为转移的好发部位,尤其右膈下淋巴丛密集,故最易受侵犯。血行转移相对少见,终末期可转移到肝、肺及脑等部位。
