心内膜心肌活检


心内膜及心肌活体组织检查。自1962年在临床开始应用，作为心脏病的诊断手段之一。其操作方法采用经皮穿刺法，若取右心室心肌可选股静脉，颈内静脉或肘前静脉，如取左心室心肌，则选股动脉插入带有活组织检查钳的心导管，在X线透视下将检查钳送到心腔内，并顶在心壁上，在左心室可顶在游离壁或间隔部，右心室则顶在间隔部，再通过心腔内心电图检查，确定检查钳所在的位置，咬取心内膜和心肌组织，通过活组织作切片检查，有助于心内膜和心肌病变的诊断。目前我国多采用右心室的心肌活检。其合并症严重者可能有心脏破裂、心包填塞，故临床应严格掌握适应症。
