麻醉、切口及游离精索均与斜疝修补术相同。步骤：①处理疝囊。疝囊较小者，不必处理；较大者，将疝囊切除，以丝线严密缝合腹膜及腹横筋膜。②修补腹膜沟管。采用Bassini或Halsted疝修补术，以增强腹股沟管后壁。巨大直疝或腹股沟管缺损过大者，可视情况采用McVay疝修补术或疝成形术。
