脊柱结核并发截瘫
paraplegia from spinal tuberculosis，Pott paraplegia
系结核性物质，如脓液、干酪样物、肉芽、死骨、坏死的椎间盘、增厚的硬脊膜、椎管内肉芽组织纤维化，或者脊柱后凸畸形或病理性移位形成的骨嵴对脊髓直接压迫或者磨损，或者脊髓的营养血管栓塞继发脊髓变性或软化等造成。其发生率约10%。胸椎结核最多见，颈椎次之，其次为颈胸段和胸腰段。一般先出现运动功能障碍，继而感觉功能障碍，而大小便功能障碍最迟出现。腰椎穿刺可发现脑脊液完全性或不全性梗阻，脊液色黄、蛋白含量增加。CT或MRI检查可进一步明确病灶及脊髓情况。在短期抗结核治疗后施行病灶清除减压术。结核性物质和死骨等压迫所致者疗效较好，而后两种原因所致者疗效较差。
