骨髓移植中最常用的一种方法。是指供者-受者间主要组织间相容性抗原配合的同种不同个体间的骨髓移植。主要在同胞兄弟姊妹间进行。将同胞兄弟姊妹骨髓组织中的造血干细胞，通过一定途径移植至病人的骨髓，重新建立骨髓的造血功能和免疫功能。对严重联合免疫缺陷综合征，重型再生障碍性贫血和急性白血病等有一定疗效。
