由于椎间盘退变加上慢性损伤引起椎间盘纤维环破裂，髓核向椎管方向突出压迫脊神经根或马尾神经。表现为腰痛、向下肢放射或间歇性跛行。腰部变直或侧弯，活动部分受限，腰及坐骨神经行径压痛，直腿抬高试验阳性，足背皮肤感觉减退，伸■肌力减退或跟腱反射减弱，少数有肌肉瘫痪。腰椎管造影、CT及MRI可知椎间盘突出部位、大小。牵引、推拿、按摩、卧床休息无效可作硬膜外封闭。保守治疗无效可手术摘除突出髓核，经皮穿刺刮除椎间盘或髓核溶解方法部分有效，但未广泛应用。
