后伸暴力引起枢椎齿状突骨折、寰椎向后移位或黄韧带挤入椎管内，如在中、下段颈椎，则黄韧带被皱褶向前压迫脊髓，椎间盘也可向后突出，椎体可向后移，尤以老年人多见。治疗主要是颅骨牵引，复位后石膏固定6周，如不能复位则作手术复位和融合植骨。
