是指颈椎1、2活动不稳定，在屈伸中超出正常范围，X线片见环齿间隙增宽(成人不超过2.5mm，儿童不超过5mm)。病因有两方面，一是咽喉炎、感染、青少年型类风湿等致C1、2周围松弛、无力；二是先天畸形，如齿状突异常、颅底部压迹或软骨发育不全、脊柱骨骺发育不良、Morquio综合征、Larsen综合征。如出现神经受压症状时，需牵引复位及C1、2稳定手术。如C1有畸形则行枕-C2融合术。
