儿童脑死亡
包括脑干在内的全部脑组织的不可逆性损害。非脑组织的功能因人工通气和药物支持有可能维持数小时至数周。脑死亡诊断标准涉及体征、脑电图、脑血流图和脑代谢等方面。参照我国成人和美国哈佛医学院(1968）、Johns Hopkins医院(1984）的诊断标准，结合儿科特点及我国国情，经1989年9月丹东全国儿科重症疾病急救学术研讨会和1989年10月长沙中华医学会第十次全国儿科学术会讨论，建议临床试用标准如下：①持续深昏迷，无自主运动，对外界刺激无反应；②经反复停机试验证实无自主呼吸，(停机试验：在充分供氧，并使PaCO2＞0.8kPa(60mmHg）时，停止机械通气，观察患儿有无自主呼吸出现，观察时间约3分钟，若心率明显减慢，立即停止试验）；③瞳孔放大、固定、对光反射消失、角膜反射消失；④心率固定，对任何刺激无反应，包括静脉注射阿托品；⑤排除低温(肛温≤35℃）、麻醉剂、肌松剂、大剂量镇静剂及严重代谢和内分泌紊乱所致的假象；⑥有条件时可做下列检查：娃娃眼试验阴性；前庭冷水试验阴性(鼓膜完整情况下，每侧外耳道注4℃冰水10ml不能引起眼震）；脑电图持续30分钟呈等电位，即使增益4倍也无脑电波出现；⑦一般需观察24～48小时。以上条件均具备，才能最后确诊。
