前置胎盘的超声所见


胎盘的位置低于胎先露部,称为前置胎盘。采用超声检查前置胎盘准确率很高,目前几乎代替了阴道检查和X线检查。检查前要求孕妇适当的充盈膀胱,目的是观察胎盘下缘与子宫内口之间的关系。探头采用纵、横、斜切联合使用,其中正中纵切很重要。由于在妊娠中期对胎盘位置随着妊娠周数增加,子宫下段形成而会向上移动,故诊断前置胎盘应在孕28周以后,至于超声所见胎盘位置以诊断前置胎盘的不同类型与临床分类同,即中央性前置胎盘:子宫内口均被胎盘完全覆盖;部分性前置胎盘:胎盘下缘部分覆盖子宫内口;边缘性前置胎盘:胎盘下缘抵子宫内口边缘。低置胎盘:胎盘部分分布于子宫下段,但尚未到达宫颈内口。
