    由于垂体前叶激素分泌过少而引起的疾病。病因①垂体缺血性坏死,常见分婉时发生大出血引起休克所致的希恩氏综合征。有微血管病变的糖尿病患者可在妊娠期不伴大出血的情况下发生垂体缺血性坏死。②垂体及下丘脑肿瘤,常见成人无功能性垂体瘤和儿童颅咽管瘤等。③垂体及下丘脑发育异常,可伴其他中线畸形。④自身免疫病、感染、肉芽肿、转移癌或全身性疾病(如白血病)累及下丘脑和垂体。
    临床表现  决定于发病年龄、激素种类、损害程度和原发疾病性质。早期症状较轻,晚期症状包括三大症群:①垂体激素分泌障碍,激素分泌减少引起相应靶腺功能减退和萎缩。生长激素(GH)分泌减少可导致垂体侏儒,患儿生长发育迟缓,身材矮小;促性腺激素分泌减少可造成儿童性发育障碍和成人的性腺功能低减、闭经不育等;促甲状腺激素分泌减少可造成畏寒、淡漠、嗜睡、便秘、智力下降、心动过缓和粘液性水肿等甲状腺功能下降的临床表现;促肾上腺皮质激素分泌减少可造成乏力、头晕、恶心、纳差、消瘦、肤色浅淡、血压下降、抵抗力减退等肾上腺皮质功能低下的临床表现。②垂体前叶功能亢进症候群。见肢端肥大症。③垂体周围组织受压症候群。可出现头痛;视力减退、视野缺损、视神经萎缩;嗜睡、不规则顽固性发热、多食和厌食等(下丘脑症群);海绵窦综合征;脑脊液鼻漏。
    诊断 依据以下几点:①垂体前叶激素及相应靶腺激素分泌不足引起的临床表现及水、盐代谢平衡的紊乱。②垂体激素水平低下而且对各种刺激试验反应很差;靶腺激素水平虽低但接受刺激后有反应(常为延迟反应)。③影像学检查结果。
    治疗 其原则是:①要长期补充替代生理剂量的下丘脑、垂体或靶腺激素,遇有应激情况时应该适当增加剂量。②对症给予支持疗法,尤其是迅速纠正各种代谢紊乱。③去除病因,恢复正常垂体功能状态。④内分泌腺体、组织或细胞移植,尚在研究之中。
