腹会阴切口后尿道吻合术
手术名。适于 膜部尿道断裂、球膜部尿道断裂及前列腺部膜部球部混合型断裂并发尿生殖膈撕裂，会阴部 血肿及尿外渗者。硬膜外麻醉，截石位，于会阴部“∩”切口，清除血肿，显露球海绵体肌 ，自尿道外口插入导尿管寻及尿道远侧断端。经下腹正中切口，清除耻骨后膀胱周之血肿及 尿液，膀胱内用尿道探子经膀胱颈插入尿道，于会阴部切口内寻及尿道近侧断端，分离近远 侧尿道并修补，使之在无张力下吻合；全层缝合尿道背侧壁，将F16—18号导尿管经 吻合口插入膀胱，缝合尿道侧壁及腹侧壁。耻骨后置烟卷引流，会阴部皮片引流并耻骨上膀 胱造瘘。术后置导尿管，抗生素预防感染，雌激素及镇静剂避免阴茎勃起。48～72小时拔除 引流条，7～14天拔除导尿管，耻骨上造瘘管可在12～14天拔除。术后定期扩张尿道。
