系指剥除壁层及脏层胸膜上的纤维板，使肺组织从纤维板的束缚中游离出来，重新扩张，胸壁也恢复呼吸运动，既能改善肺功能，又可免除胸廓畸形的一种最理想的手术。适用于病程不长、肺内无病变、能复张的慢性脓胸病例。常用侧后胸切口，切除一根肋骨，经肋床进胸后，作胸膜外剥离，剥下壁层纤维板，随后剥离脏层纤维板，将整个脓囊袋切除。术毕在前胸第2肋间及侧后第6～7肋间各放置引流管，按负压吸引使肺扩张。
