胸膜内胸廓成形术
intrapleural thoracoplasty
指切除患部肋骨，并同时切除壁层胸膜纤维板，使胸壁塌陷，压缩消灭脓腔的胸廓成形术。过去采用Schede术式，将脓腔顶部的肋骨、肋间组织、壁层胸膜及增厚的纤维板一并切除，使胸壁剩留的软组织下陷，消灭脓腔。此术式损伤大，造成胸壁软化，反常呼吸。现在多采用改良的胸膜内胸廓成形术，仅在骨膜下切除脓腔顶部相应的肋骨和壁层胸膜纤维板，清除脏层胸膜上的肉芽组织，将肋间束排列固定在脏层胸膜纤维板上。适用于慢性脓胸或结核性脓胸，肺内有活动性结核病灶，有支气管胸膜瘘者。
