适应症:
1.适用于器官移植时的抗免疫排异治疗。用于人的同种异体移植有明显疗效,特别是对肾脏移植的病人。但也有一定局限性,主要是对急性排异期有效,对体液免疫所致的超急性排异无效。与硫唑嘌呤、泼尼松合用可提高脏器移植的成功率。骨髓移植时,供者与受者双方在术前均给以抗淋巴细胞球蛋白,有防止移植物抗宿主反应的作用。 2.自身免疫性疾病。ALG对肾小球肾炎、红斑狼疮、类风湿性关节炎、重症肌无力等自身免疫性疾病有良好疗效,对顽固性皮炎、脉管炎、原发性肝炎、交感性眼炎等也有一定疗效
用量用法:
肌注:马ALG每次每千克体重4~20mg,兔ALG每次每千克体重0.5~1mg,1日1次或隔日1次,14日为1疗程。 静注:马ALG每次每千克体重7~20mg,稀释于50~100ml等渗盐水中,于4~6小时内滴完,输入适量等渗盐水,1日1次。
注意事项:
1.肌注可引起局部疼痛、红肿、发热、荨麻疹等,甚至有过敏性休克,静注也可有短时高热(38~40℃,持续3小时)、发冷,有时伴有关节痛和气短。静滴可见一过性体温升高与寒颤、低血压、心率增快等。一般在1~2小时内消退。 2.过敏体质者禁用,有急性感染者慎用。 3.采用ALG治疗自身免疫性疾病应特别慎重,因长期应用使机体的免疫监管功能降低,给癌变细胞的发展以可乘之机。
