  由于主动脉瓣和肺动脉瓣区的关闭时间明显不同步(超过0.035秒)所致,在肺动脉瓣听诊明显。在健康儿童和青年可听到,尤以深吸气末更明显。在病理情况下,任何原因引起一侧心室排血量过多或排血时间延长时,均可使第二心音分裂。常见于二尖瓣狭窄或房间隔缺损,亦可见于右束支传导阻带、动脉导管未闭、原发性肺动脉扩张等。因右心室排血时间延长致肺动脉瓣关闭明显延迟所致。在主动脉诊断狭窄或左束支传导滞时,主动脉瓣关闭音可发生在肺动脉瓣关闭之后,这称为第二心音逆分裂。此时,吸气时分裂互相接近甚至消失,而呼气时则明显。房间隔缺损时,第二心音分裂程度,几乎不受呼气、吸气时相的影响,称为第二心音固定性分裂。
