健康保险者名单制度
健康保险者名单制度又称按人头付费和按病种付费。按月或其他特定时间(通常为一年)，医疗服务机构根据医生服务的人数向健康保险管理者收取固定数量的费用的一种制度。在此期间，医生在提供合同规定的一切医疗服务时不再收费。许多国家侮个医生一般负责向2000一3000个有健康保险者提供健康服务。为保证医疗服务质量，按人头收费通常规定每个医生负责服务人数的最高限额。按人头付费制是一种预付制，其特点是医生收入与服务人数成正比。服务人数确定后，收入固定，服务项目越多，支出越多，收入反会减少。因此，实行按人头付费，能防止医生提供过多的服务，有利于控制成本和费用;能鼓励新毕业的医生到缺医少药的地方工作，有利于促进开展预防工作，减少医疗服务工作量和降低医疗费用支出。实行按人头付费，对病人来说，可根据与保险机构签约的医生名单自由选择医生。 按病种付费，又称疾病诊断相关分组。即根据国际疾病分类法，将住院病人疾病按诊断分为若干组，每组又根据疾病的轻重程度及有无合并症、并发症分为若干等级，对每一组不同等级分别制定价格.按这种价格对该组某级疾病治疗服务的全过程一次向医院支付费用。按病种付费制是世界上较先进的付费方法之一，其特点是:通过统一的疾病诊断分类规定支付标准，有利于加强医疗服务费用的管理，控制医生过度提供医疗服务，使某一特定疾病的费用标准化。
