Methods A total of 465 patients with teratoma, among whom 13 with immature teratoma and 3 with malignant change in benign ovary teratoma, were enrolled in the study from 2000 to 2005.

目的探讨电视腹腔镜下手术治疗卵巢良性畸胎瘤的效果和手术技巧,防治术中囊肿破裂等并发症发生。

卵巢未成熟型畸胎瘤
在卵巢恶性生殖细胞肿瘤中居第3位，占卵巢畸胎瘤的1%～3%。年轻患者较多，平均年龄17～19岁。单侧居多，最大直径可超过20cm。包膜光滑，也可自行破裂。切面囊实性，内有鱼肉样出血坏死。除由3个胚层束的成熟组织如毛发皮脂腺等外，还有未成熟组织，特别是神经上皮组织。Norris根据此点将此瘤分成三级，对治疗及预后均有关。Ⅰ级：未成熟组织少，神经上皮1个/低倍视野。Ⅱ级：较多未成熟组织，神经上皮1～3个/低倍视野。Ⅲ级：多量不成熟组织，神经上皮4个或更多/低倍视野。由于转移发生较多，治疗应根据分期进行手术；年轻患者要求保留生育功能，可根据分期进行单侧附件切除。术后应辅以化疗。有报道复发瘤经过化疗有从未成熟型向成熟型逆转的趋势，提示对于此类患者应积极治疗。
