| 阵发性睡眠性血红蛋白尿 paroxysmal nocturnal hemoglobinuria 获得性的细胞膜缺陷引起的一种溶血性贫血。主要是细 胞膜上的衰变加速因子（DAF）和同种限制因子（HRF）缺陷，以红细胞更明显，因而对血浆中的补体异常敏感而发生血管内溶血，由于PNH是基因突变引起的干细胞异常，所以白细胞和血小板计数亦可减少。病人于睡眠后第一次尿可呈酱油色，少数病人有血栓形成，化验除有溶血的一般阳性结果外，蔗糖水试验和酸溶血试验阳性可确定诊断 。尚无特效疗法，主要是对症治疗 ，贫血严重者可输生理盐水洗涤的红细胞，若输全血可能会促进溶血。也有骨髓移植治愈PNH的报道。 |
