胃十二指肠溃疡急性穿孔
acute gastroduodenal ulcer perforation
是活动性胃十二指肠溃疡逐渐侵蚀、穿破浆膜的结果。多为幽门附近的胃或十二指肠球部前壁穿孔。根据溃疡病病史，近期症状加剧，突发持续性上腹剧痛，明显的腹膜刺激征，甚至腹肌板样强直，肝浊音界缩小或消失，X线检查有气腹，可确定诊断。必要时可行腹穿。不典型者需与急性胰腺炎、急性阑尾炎等鉴别。对全身情况差、穿孔时间超过12h，腹膜炎局限及空腹穿孔者可试行非手术治疗。其余病例需急诊剖腹手术，行单纯穿孔修补术或治愈性手术。
