| 局限性肠炎 regional enteritis 原因不明的慢性炎症性肠病。又称节段性回肠炎、克罗恩氏病、克隆氏病。欧美国家多见，白种人居多，犹太人更为多见。病因尚不清楚，可能与免疫、遗传、精神和感染因素有关。病变可以侵犯胃肠道的任何部分，但主要累及远端小肠和结肠。病理特征为胃肠道非干酪样肉芽肿合并溃疡，呈节段性分布。早期常无症状或症状轻微，随着病变发展，症状逐渐明显。腹痛是最常见的症状，其次可以表现腹泻、发热、腹部肿块、瘘管形成和肠梗阻。本病除具有胃肠道症状外，还可具有口腔溃疡、关节炎、眼虹膜睫状体炎、结节性红斑等与免疫有关的肠道外表现 。X 线钡剂灌肠和结肠镜检查可以帮助诊断，尤其结肠镜检查时取活体组织做病理学检查更为可靠。一般以内科药物治疗为主，如应用水杨酸偶氮磺胺吡啶或激素。本病易复发。若有合并症时可考虑手术治疗。 |
