纵隔的位置发生改变，X线表现为纵隔不同程度地向一侧移位，此种移位比较固定而不受呼吸影响。常由于胸腔积液、气胸、单侧肺气肿及一侧膈肌升高明显等，使患侧胸腔压力增大，推压纵隔向健侧移位。亦见于肺不张、广泛性胸膜肥厚、胸廓成形术后等，纵隔被牵引向患侧移位。
