性质:又称豆中毒。是较常见的性连锁遗传缺陷,由于红细胞内葡萄糖6-磷酸脱氢酶(G6PD)缺少,进食蚕豆、某些药物或接触蚕豆花粉后出现严重的溶血性贫血,故称蚕豆病。G6PD催化6-磷酸葡萄糖脱氢生成6-磷酸葡萄糖酸,同时NADP+还原为NADPH。红细胞内NADPH的主要作用是作为谷胱甘肽还原酶的辅酶,使氧化型谷胱甘肽(GSSG)还原为还原型谷胱甘肽(GSH)。GSH系红细胞内主要抗氧化剂,是维持红细胞正常结构的决定因素。G6PD缺少,红细胞内GSH水平低下,进食蚕豆后可能由于蚕豆中的香豌豆嘧啶和异尿咪自氧化生成醌类化合物和H2O2,使红细胞内GSH更为不足,含巯基的膜蛋白、酶和血红蛋白得不到保护,生成的氧化产物使红细胞变形,导致溶血。香豌豆嘧啶的自氧化产物还可引起红细胞内钙、钾代谢紊乱,加重红细胞的破坏。
